Be the Nurse Every Senior Community Wishes They Had.

At Auburn Crest Primary & Palliative Care, we bring healthcare directly to seniors living in assisted living and memory care communities throughout the Treasure Valley.

We're looking for an experienced, relationship-focused LPN to serve as the clinical bridge between our providers, community staff, residents, and families.

Rather than spending your day on a hospital unit, you'll support a designated group of senior living communities, helping residents receive exceptional care where they live.

If you enjoy educating caregivers, solving problems, coordinating care, and building meaningful relationships, this role offers the flexibility and purpose many nurses are searching for.

What Makes This Role Different

  • Flexible scheduling with greater work-life balance than traditional hospital nursing
  • Consistent daytime hours
  • No hospital floor assignments
  • Opportunity to become the trusted clinical expert for your communities
  • Meaningful relationships with residents and families
  • Autonomy to manage your day while supported by an experienced interdisciplinary team

What You'll Bring

  • Active Idaho LPN license
  • Current CPR/BLS certification
  • Strong clinical judgment and critical thinking skills
  • Excellent communication and relationship-building abilities
  • Exceptional organization and time-management skills
  • Passion for serving seniors and improving quality of life
  • Experience in assisted living, memory care, geriatrics, hospice, palliative care, home health, or primary care preferred
  • Reliable transportation for local travel throughout your assigned territory

Every day is worth celebrating.

At Auburn Crest, we believe seniors deserve exceptional healthcare delivered with compassion, dignity, and consistency. Join a team that's redefining how care is provided in assisted living and memory care communities throughout Idaho.

Apply today and help bring exceptional care directly to where seniors call home.#ACH
